ALT and AST: what your liver function test means

ALT and AST are enzymes found mainly in the liver. When liver cells are stressed or damaged, more of these enzymes leak into the blood, so higher values can suggest the liver is under strain. They are part of a liver function test (LFT) and are read together with other values and your history — not alone.

ALT / AST are
Liver enzymes (ALT is more specific to the liver)
Higher values may suggest
Liver cells under strain — many causes, mild to serious
India naming
ALT = SGPT, AST = SGOT

Why these enzymes show up in a blood test

ALT and AST live mainly inside liver cells. When those cells are stressed or damaged, a little more of each enzyme leaks into the bloodstream — so a blood test can pick up higher levels. That is why ALT and AST are part of a liver function test (LFT): they are an early, sensitive signal that the liver is under some strain.

Higher values: many possible reasons

A raised result does not point to a single cause. It can follow a recent illness, certain medicines, alcohol, or a fatty liver, and sometimes something more serious. The degree of elevation, the ratio between ALT and AST, and your history all shape what it means — which is why doctors never read these two numbers in isolation.

Questions to ask your doctor

Bring these to your next visit. They are written to help you understand what was already decided — not to second-guess your doctor.

  1. 1.How far outside the range are my ALT and AST, and does that level concern you?
  2. 2.Could a medicine, alcohol, or a recent illness explain this?
  3. 3.Do these need to be repeated, and when?

Common questions

What does it mean if ALT and AST are high?
Raised ALT and AST commonly suggest the liver is under some strain, because these enzymes leak into the blood when liver cells are stressed. Causes range from harmless and temporary (a recent illness, some medicines, fatty liver) to more serious. The degree of elevation and the full picture decide what it means — ask your doctor.
Are SGPT and SGOT the same as ALT and AST?
Yes. SGPT is the older name for ALT, and SGOT is the older name for AST. Indian lab reports often use SGPT and SGOT; they measure the same enzymes.
